Apropos above, I/we hereby request you to kindly disconnect broadband services for my telephone no. 23666* with immediate effect. As i no longer require this service.

I can assure you that upon disconnection all pending accounts will be closed.

Kindly note further billing will not be paid by me from the date of submission of this application along with depositing modem and accessories.

Please do the needful at earliest else we will not be liable for nonpayment of any unnecessary bill raised due to your delay.
